What is an Accident?

An accident is the final event in an unplanned process that results in injury or illness to an employee and possibly property damage. It is the final result or effect of a number of surface and root causes.

An “event,” occurs when one “actor” (one person/thing) performs an “action” (does something).

A person or thing (equipment, tools, materials, etc.) will do something that results in a change of state.

An accident may be the result of many factors (simultaneous, interconnected, cross-linked events) that have interacted in some dynamic way.

The law requires organisations to be very responsive when incidents takes place and are awfully specific on the process that should be followed. Knowing how to manage incidents as required requires diligent training and execution. This training course is designed with this purpose in mind.

Relevant current legislation, regulations and directives pertaining to occupational health and safety.

Mandatory Codes of Practice.

South African National Standards and other relevant Standards.

OHS Act requirement for reporting and investigation of workplace incidents

Procedure in incident occurrence:

securing the scene,

handling of injured persons,

handling of fatalities,

dealing with work processes

initial reporting.

 Consequences of non-compliance with legal/ organisational specific requirements

The investigation phase

Purpose, type and extent of information required

Resources required to conduct investigations: documentation, human resources, tools and materials.

Information is gathered with accuracy and relevance in line with policy

Post Investigation Phase

Reports completed in line with requirements

Reports communicated to relevant designated persons

Physical evidence and resources used for investigation are processed
